A unique opportunity to manage a community-based cardio-pulmonary prevention and rehabilitation service in Milton Keynes. Based at our Cardio-pulmonary rehabilitation centre, we have 350 members. On joining our service they undergo either a six-week CCG commissioned Phase IV cardiac rehabilitation or a unique pulmonary and heart failure programme. Once completed they can join one of 8 weekly maintenance classes. Leading a team of qualified Exercise Instructors and Health Professionals, you will be responsible for the day to day running of the centre, ensuring that it meets the needs of our service to our members with established cardiovascular or pulmonary disease and heart failure. Responsible for all clinical aspects of exercise rehabilitation and service development, you will make clinical assessments of high-risk patients referred to the Phase IV programme or Pulmonary/Heart Failure rehabilitation. This is essential to ensuring safe exercise limits and effective exercise outcomes for participants. With the Officers and management team you will set the strategic direction for the charity’s exercise services and support programme to deliver agreed KPIs. You will liaise with colleagues in primary and secondary care and be an ambassador for the service.
