Head of Cardiovascular Health Programmes
Croi, the
Cardiac and Stroke Foundation, which is an independent Irish not-for-profit
organisation, seeks to appoint a Head of Cardiovascular Health Programmes,
based in its centre in Galway.
The Croí Heart & Stroke Centre in Galway is the only centre of its kind in
Ireland, and is one of only eleven centres in Europe to have achieved the
European Association of Preventive Cardiology Accreditation as a Centre of
Excellence in Cardiovascular Risk Management. The community based centre has a
multidisciplinary health team who deliver a range of evidence based prevention,
risk factor management, recovery and cardiovascular health promotion programmes
for those who self-refer or who are referred by their GP or hospital physician
across the areas of heart disease, stroke, diabetes and obesity.
This is an exciting opportunity to lead, manage, develop and deliver a
portfolio of cardiovascular awareness, education and training activities which
proactively respond to the needs of the community, especially those most at
risk, underserved or disadvantaged. A key component of this role is responding
to the 'new normal' of using digital and virtual communication channels.
Job Summary:
We are seeking to appoint an exceptional, hands-on, action orientated
individual, who will have strategic and operational responsibility for the
development (new and existing) and delivery of all Croi Health Programmes.
Working within an entrepreneurial and dynamic environment, the Head of
Programmes will lead and develop a multidisciplinary team of individuals who
are passionate about prevention, recovery, and the maintenance of
cardiovascular health. He/she will be a key member of the organisations senior
leadership team.
He/she will be responsible for:
- the day to day management, delivery
and quality control of a diverse range of evidence based, outcome-focused
health and lifestyle programmes most of which need to be delivered virtually
and using new technologies.
- the day to day leadership and management
of a multidisciplinary team, ensuring effective team work, team development and
inspiring excellence in person-centred service delivery.
- initiating and leading the
development of new programmes, campaigns and activities, especially, peer-led
and self-management programmes across the areas of heart disease, stroke,
obesity and diabetes, incorporating the use of new digital technologies.
- working with the communications
team on all matters relating to health programmes e.g.; media releases, patient
resource materials, reports and publications, health and awareness campaigns.
- working with the fundraising team
in preparing project proposals and engaging with volunteers, sponsors &
donors.
- proactively establishing, developing
and managing collaborative relationships and partnerships across all
stakeholders.
- playing a leadership role in
establishing sustainable funding models to ensure delivery of programmes in
line with Croi's overall strategic direction.
- facilitating and supporting
engagement in research, recording and measuring programme outcomes, preparing
and dissemenating reports and publications.
- playing a leadership role in the
annual strategy review, planning and budgeting process
- preparing and writing grant
proposals as well as monitoring, reporting and presenting outcomes of grant
funded activity.
- reporting to the CEO and
occasionally to the board of directors of Croi
- taking on additional related projects;
representing Croi at events as required; attending meetings and making
presentations.
The above covers the most significant responsibilities of this position. It
does not, however, exclude other duties, the inclusion of which would be in
conformity with the seniority of this role.
Qualifications:
· Relevant healthcare qualification (e.g.
preventive cardiology, public health, nursing, health promotion, physiotherapy,
psychology, dietetics) with a minimum of 4+ years leadership experience, in the
area of healthcare, especially cardiovascular disease or chronic disease
management.
· Knowledge and understanding of
current trends and developments in cardiovascular healthcare, especially
chronic disease prevention and rehabilitation
· Driven to push the boundaries of
digital engagement in preventive healthcare, harnessing the power of digital
channels and technologies to improve the cardiovascular health and wellbeing of
individuals, families and communities.
· Demonstrated project/business
management experience and leadership in developing and implementing healthcare
programmes or initiatives in a service-driven, outcome focused
environment.
· Proven experience of leading,
developing and managing teams and a track record of building and cultivating
collaborative and long-term relationships across multiple stakeholders.
· A highly motivated self-starter
with exceptional communication and interpersonal skills, who thrives in a
fast-paced environment; detail oriented with the ability to plan and execute
complex initiatives with a solutions-oriented approach.

Remuneration:
· Salary commensurate with
experience.
